HEALTH CLASS INFORMATION SHEET

Daily Classroom Routine & Instructions

1. Be in your seat with your journals, and ready to work before the bell rings
2. Begin your journal writing warm up assignment
3. The writing assignment ends when our daily lesson or activity is ready to begin
4. At this time, I’ll ask for and collect homework if needed
5. Be prepared to discuss your daily journal assignment as our lesson is beginning
6. Begin daily lesson or activity – lessons and activities will be written on the board
7. Wrap up and Review – discussion and reflection time – clean up if necessary
8. Time for questions, comments, or concerns – Homework or Test reminders
9. Organize yourselves and your materials and prepare to be dismissed

ALL ASSIGNMENTS AND JOURNAL ENTRIES MUST INCLUDE THE FOLLOWING: NAME, DATE, CLASS PERIOD, and TITLE IF REQUIRED

* Classroom agendas (assignments & lessons) will be written on the board and posted on my webpage on a weekly basis. All students will need to write down the weekly assignments and lessons in his or her required moose binder calendars. *


Health Grade Determination

PART ONE – Daily Grades – 50% of your six weeks grade

The following are examples of classroom daily grades:
• Current events
• Small and large group activities
• Worksheets/Test reviews/Quizzes
• Participation

PART TWO – Test Grades – 50% of your six weeks grade

Tests
Major Projects – (TBD)

PART ONE + PART TWO = Six Weeks Grade


SEMESTER AVERAGE – Combination of the following:

1st six weeks grade
2nd six weeks grade
3rd six weeks grade
Semester Exam – 14% of semester grade – consisting of completed journal entries and a comprehensive reflective essay – (7th graders will also have a folder project at the end of the semester)

Group work
At times, you will work in small groups, but be asked to turn in an individual assignment for an individual grade. Your individual assignment should be an original thought that you created using ideas discussed by the group, as well as your own ideas. At other times, while working in large groups, your group will turn in a group assignment for a grade. The group assignment should be an original thought or idea that the members of your group collaborated and worked together on in order to produce an overall group grade.
If your group seeks your help, you should help with a POSITIVE attitude.
If you have questions, ask your group members first.
If your whole group is confused about something, formulate one or two focused, key questions to ask me.
Homework

The majority of our work will take place inside the classroom. The busier the students stay, then less outside activities or homework they will have. Homework that is assigned and which has a due date must be brought back to class and finished on that date. I do not have a box or a file for students to place their homework in. I will collect and distribute their homework when ever the time may arise. By doing this, I know I can alleviate any miscommunications between myself and my students. On an occasion when our class has homework due, students should be prepared to hand in their homework during the time in which they are working on their journal writings, but before I start that day’s lesson.


Late Work

Most of the work we do in this class will take place during the class period. If and when homework is assigned, that homework assignment is to be turned in on time. If work is not received on the due date, it is considered late. Late work is accepted using the following grading scale per LDMS policy:

• 1st Day Late – Maximum grade of 70
• 2nd Day Late – Maximum grade of 50
• 3rd Day Late – No credit = 0


Make up for Absences

It is the student’s responsibility to make up his or her own work for absences. A student has the amount of days missed to turn in any activity, assignment, or test they may have missed. After those days have passed, zeros will be assigned for that student’s missing grades. Our weekly health assignments and agendas will be posted on the board for the students to copy down in their notebook calendar every Monday. So, each student should have a pretty good idea and understanding as to what is due, what the class will be focusing on, and what will be expected of them during that week.

Test – A retest will not be given for grade dissatisfaction, but alternate make up tests can and will be given to those students with excused absences.

Health Folder Requirements – 7th graders only – HS students will have alternate assignment

All folders MUST be kept clean, neat, and organized, in the exact order listed below.
Each student will be required to keep their Health folders in their LDMS notebook or in an assigned location in the classroom.

5. Homework
6. Any writing assignments and/or any individual or group activities
7. Current Events
8. Test Reviews

10. Completed Journals - to be turned in with your folder

* Each class will have a journal that will need to be placed and kept in an assigned tub at the end of the class period*
